Cityblock Health has finalized its acquisition of Homeward Health through an all-stock transaction, while simultaneously closing an $116 million Series E funding round co-led by General Catalyst. The move extends Cityblock’s footprint into rural healthcare delivery and strengthens its commitment to value-based care models covering medical, behavioral, and social needs.
Homeward, established in 2022 by former Livongo leaders including CEO Jennifer Schneider, M.D., began operations with a $20 million seed investment from General Catalyst. The company’s framework merges digital tools with hands-on care, initially targeting primary and specialty services in underserved regions—with cardiology as its first focus. Through remote monitoring, mobile clinics, and home visits, it intervenes earlier in patient trajectories, such as identifying heart failure risks before hospitalizations become necessary.
The startup has since secured over $70 million in total funding and forged partnerships to serve Medicare patients in Michigan and Minnesota. Its rural specialization includes collaboration with insurers like Blue Cross Blue Shield of Michigan, a model Cityblock will incorporate alongside its existing urban Medicaid and dual-eligible programs.
Cityblock’s CEO, Toyin Ajayi, M.D., described the acquisition as a strategic alignment in a company blog. With nearly 200,000 members and annual revenue of $2.2 billion, a 77% increase from the prior year, the platform bridges urban and rural healthcare gaps. The deal supports broader efforts to enhance efficiency in government-funded healthcare systems, where budget limits and workforce shortages remain persistent challenges.
Homeward’s methodology contrasts with conventional reactive care by prioritizing preventive strategies and community-based interventions. This approach aligns with the growing emphasis on value-based models, where proactive measures lower long-term expenses while delivering better results for at-risk populations.
Cityblock’s recent entry into Long-Term Services and Supports (LTSS) across its 11 operating states reinforces its role as a national healthcare provider. The merged organization now targets the 120 million Americans relying on taxpayer-funded healthcare, tackling disparities in both urban and rural service availability.
The integration follows Cityblock’s earlier expansion into behavioral health and social determinants of health, areas where it has developed proprietary technology to streamline care coordination. Homeward’s rural expertise complements Cityblock’s urban operations, creating a unified platform to address fragmented healthcare delivery nationwide.
